Restorative Aide CNA

Encore Parkside is looking for a dedicated Restorative Aide to support residents in maintaining and improving their functional abilities. RESPONSIBILITIES: · Performs restorative treatments under the direction of the Director of Nursing · Collaborates with nursing staff to ensure that residents are available for the restorative programs, when necessary, will assist the nursing staff with direct care. · Documents all restorative activity performed in EMR. Completes and maintains documentation according to Rehabilitation Restorative program guidelines and facility policy. · Reports and documents residents’ significant changes to assigned CNA, Nurse and Restorative Nurse. · Updates and maintain quarterly restorative progress notes for the restorative nurse and rehab nurse. · Consults with unit staff routinely to be aware of resident diagnosis, precautions, and contraindications to performance of various restorative treatments. · Recognizes and treats emergency situations in accordance with facility emergency policies. · Transports residents, equipment, and supplies necessary to perform treatment. · Follows established procedures and safety guidelines pertaining to the care and use of equipment and supplies Gets training on new equipment prior to usage. · Prepares, maintains, and cleans treatment areas in preparation for direct patient care. Reports any equipment that is need of repair to Director of Nursing. · Recognizes the importance of providing Resident Centered Care. Regulatory Compliance · Respects Residents Rights for Privacy and Dignity · Reports all suspected, alleged, and/or witnessed verbal, physical, or sexual abuse immediately to supervisor.(PHI) Leadership · Serves as a role model and instructor for C.N.A.’s in demonstrating proper rehabilitation technique during delivery of resident care. Strives to develop a cohesive team with nursing staff. · Able to clearly communicate and direct residents in performance of restorative program. · Monitors CNA for compliance with restorative programs assigned to them. QUALIFICATIONS/REQUIREMENTS Licensure/Certification · Current State of Delaware C.N.A. certification. Education · Certificate demonstrating completion of an IDPH approved Occupational Rehabilitation and Physical Rehabilitation course. · Current CPR card. Experience · At least 1 year experience of long term care nursing as a C.N.A. · May be newly certified in Occupational Rehabilitation or Physical Rehabilitation. ENVIRONMENATL AND PHYSICAL DEMANDS: Physical Activities: Constantly standing, walking, talking, hearing, squatting, bending, squatting, reaching, twisting, driving, feeling, and grasping. Occasionally sitting, crouching, crawling, reaching hands and arms, tasting, and smelling. Range of Motion: Constantly moving from waist-shoulder height and waist and below. Job Description Nemours is seeking Nursing Assistant to join our team in Wilmington, DE for our Short Stay Observation Unit. Position is full-time, 60 hrs every two weeks, rotating shifts, with weekend and holiday rotations. 4 West is a 24-four-bed limited-stay unit that typically cares for general medical surgical patients whose care is driven by protocols. The unit consists of single patient care rooms, which can also be isolation rooms. The goal of the unit is to provide very high-quality, high-value patient care within a 72-hour time frame. Each patient room is equipped with oxygen, compressed air and suction, along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the health care team to access a patient’s electronic medical record. These computers are also used in the barcoding administration of medications which ensures patient safety by verifying that the correct medication is given to the appropriate child. Family-centered care rounds are performed daily to assure an interprofessional approach and partnership with the patient and family The nursing assistant role is responsible for assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. Job Description Nemours is seeking a Nursing Assistant for our 3 West floor! Position is full-time - 60 hrs every 2 wks, 12-hr rotating shifts, every third holiday, two weekends out of six. This position is responsible for: Assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Job Description Our 3 East unit is seeking a Nursing Assistant to join our team in Wilmington, DE! The nursing assistant role is responsible for assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. Position is full-time, 60 hrs every 2 wks, rotating day and night shifts with weekend and holiday requirement. 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include: chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management, and central line care and management. Essential Functions Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Uses Hospital Information Systems as required for patient care, (e.g. documenting vital signs and patient care activities.) Accurately relays information from physicians, nurses, laboratory personnel and others.
