Responsibilities Provides direct care to patients in accordance with their individualized care plans Assists patient with activities of daily living, including bathing, oral care, dressing, grooming, and toileting Assist in transportation and transfer of patients, ensuring comfort and safety during movement Monitor and record vital signs, intake and output, and other patient data, reporting any abnormalities or concerns to nursing team Accuchecks completed as directed Accurate and prompt documentation Assist with patient hygiene and comfort measures, including turning and repositioning patients to prevent pressure ulcers Support patients with feeding and hydration needs, adhering to dietary restrictions and assisting with mealtime activities.
