Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. II. QUALIFICATIONS A. Must exhibit a positive, enthusiastic and collaborative approach as evidenced by peer and/or supervisor support for the position. B. Must exhibit the ability to work harmoniously to engage others through a supportive approach and willingness to assist others. C. Successful completion of a nursing aide course in an accredited nurse training program as evidenced by a diploma, certificate or other written verification from the school and successful completion of the Department approved nursing assistant competency evaluation or successful completion of a United States military training program that includes the content of the Basic Nursing Assistant Training Program D. Maintains a record of results of rounds (QA Tools) and provides DON with a copy daily. D. Reports trends/patterns of safety non-compliance to the DON and nurse on duty daily. E. Reports needs for safety equipment/supplies to DON daily. F. Ensures that newly ordered safety equipment/supplies are implemented upon delivery. G. Reports potentially unsafe equipment to the nurse on duty and to the DON immediately. H. Assists with the distribution of clinical alerts to other staff that are working with certain focused residents/patients. I. Answers call lights promptly. J. Assists in the updating of the fall prevention intervention log (which includes visual interventions. K. Meets with new admissions and reviews primary safety measures. L. Reports any changes in resident/patient condition to the nurse on duty. M. Secures incident/accident reporting from the nurses upon arrival for duty. N. Checks to ensure that new interventions are promptly implemented post occurrence. O. Communicated trends/patterns, and individual resident/patient safety concerns to the IDT through Stand-Up Meeting (day and evening shifts), and during early mornings for night shift. P. Communicates with the CNAs to assists with determining the root cause or new behaviors associated with incidents/accidents, and potential new interventions based on the root cause. Communicates this information to the restorative nurse. Q. Meets with DON and corporate to review observations and recommendations for the incident/accident prevention program. R. Attends and actively participates in resident/patient care plan conferences as directed by the IDT team. S. Attends and participates in facility Incident/Accident related meetings. T. Attends educational sessions related to resident/patient safety and shares educational information with the IDT. U. The expectation is that the ideal candidates would be able to pick up 3-to-5 PM shifts per week. There will be a weekend differential in pay for those that decide to pick up a weekend shift. Scheduling is truly flexible. The Certified Nursing Assistant is a certified paraprofessional specifically orientated to assist a Private Duty Home Service client with activities of daily living in their place of residence, as specified in the individuals Plan of Care, with adherence to At Home Quality Care’s standards. Duties are limited to those in which the CNA has demonstrated competency. The CNA for Home services is directly supervised by the Private Duty Coordinator as an employee of At Home Quality Care. GENERAL RESPONSIBILITIES: · Observation of client functioning and environment status and reporting changes to the supervisor both orally and in documentation. · Carry out tasks as outlined in each individual clients Plan of Service Care. · Provide assistance with incidental household chores including initiating proper diet, meal preparation, light housekeeping, laundry and linens for client. · Work with clients, family, and agency to create and maintain a safe environment and report any hazards. · Assist client with activities outside the residence, including shopping, errands, and appointments. · Provide companionship, socialization and diversional activities including mental stimulation and emotional support. · Assist with activities of daily living and personal care with the scope of services specified by all deferral and state licensing bodies and according to agency policies, including: bathing, routine hair care, dressing, foot and nail care, routine skin care, shaving, oral hygiene, eliminations, nutrition, feeding, positioning, safe transferring and ambulation, exercise and medication reminding or prompting. · Initiate CPR, Basic First Aid, and contact EMS in case of emergency. · Maintain compliance with agency policy and procedures regarding infection control. REQUIREMENTS: · Successful completion of a Certified Nursing Assistant (CNA) course approved by the State of Illinois - preferably on the Illinois Health Care Worker Registry · At least one year of experience in home health or hospice; highly preferred. · Customer interaction/customer service experience · Excellent organizational, analytical, communication and customer service skills · Ability to take initiative and problem solve · Excellent attention to detail and follow-up skills · Criminal Background Check with no disqualifying conditions, DMV records showing a good driving record · Reliable transportation, valid driver’s license, and current auto insurance. · Current/Continuous CPR and Basic First Aid certifications. · Exhibits a positive attitude and demonstrates leadership qualities and effective interpersonal relationships with both internal and external customers. · Can work independently with minimal supervision. · Ability to lift 50 lbs. · Need to be able to stand, crouch or bend for long periods of time.
