Wound Care Monitor reactions and client progress. Coordinate client services. Notify the physician and other personnel (DON, Case Manager, etc) of change in the client’s condition. Perform skill outline in the agency’s approved policy and procedure manual. Discharge the client from skilled nursing services when the discharge criteria have been met. Case conference clinicians providing care to ensure coordination of care. Update clinical records according to policy and procedures. Update knowledge and skills by attending in-service programs, continuing education programs, seminars,and self-study programs annually. Adhere to state regulations. This key leadership role is ideal for a Registered Nurse with strong hospice or palliative care experience and a heart for end-of-life care. The DPCS ensures exceptional care quality, regulatory compliance, and team coordination while supporting patients, families, and staff in a meaningful way. Key Responsibilities Clinical Leadership: Direct the delivery of hospice services in compliance with state, federal, and internal standards. Team Management: Recruit, train, supervise, and mentor clinical staff including RNs, LVNs, social workers, and home health aides. Care Coordination: Guide the interdisciplinary team in developing and executing personalized care plans.
