At least 4 years of full-time experience within the last 5 years in hospice, palliative care, supportive care, home health, or a primary care setting. Minimum of 2 years of supervisory or managerial experience in a hospice or home health setting within the past 5 years. Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable. Valid driver’s license with a clean driving record. Reliable transportation and willingness to travel as needed. Ability to lead bereavement support groups and make sound clinical decisions.
