Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. Summary The Medical Assistant is responsible for assisting Physicians, Mid-Level Practitioners, and Registered Nurses with the evaluation, management, and treatment of patients in an ambulatory care setting. Prepares patient for exam based on reason for visit. Prepares and supports patient and assists provider with procedures. Reviews appointment schedules and ensures all patient charts are available for the visit. Reviews patient charts and ensures that all ancillary reports and required paperwork are available to the provider at the time of visit. Performs basic lab and testing procedures according to standards. Performs phlebotomy and EKGs. Administers immunizations as ordered by Physician. Administers RUMG approved intramuscular or subcutaneous medications after competency has been assessed, verified and documented. Performs nebulizer treatments in the clinic setting after competency has been assessed, verified and documented. Performs Tuberculin skin testing upon the order of the Physician. Demonstrates appropriate population specific techniques with patients. Documents administration of over-the-counter medications and/or other medications in Epic per policy or protocol. Cleans, prepares and stocks exam rooms, treatment rooms, and workstations. Calls patients regarding test results and medical instruction under the direction of a Physician. Calls in medication refills under the direction of a Physician or protocol. Documents patient care information in the outpatient medical record according to standards. Assists with scheduling of patient tests, procedures, and follow-up appointments. Assists in the care, cleaning, and sterilization of equipment and instruments. Maintains the inventory of medical and linen supplies. Maintains quality control for equipment. Provides cross coverage to other clinical areas as assigned. Participates in quality improvement projects. Maintains compliance with The Joint Commission (TJC) standards and other regulatory bodies. May have Unit specific duties as needed.
