T his position offers a Hybrid/work from home option and provides clinically based concurrent and retrospective review of inpatient medical records to evaluate the utilization and documentation of acute care services . Duties include but not limited to: Providing clinically based concurrent and retrospective review of inpatient medical records to evaluate the utilization and documentation of acute care services. Responsible for improving the overall quality and completeness of clinical documentation. Promotes a partnership between the concurrent clinical reviews, medical record coders, and physicians to improve documentation and reimbursement. Facilitates clarification and specificity to clinical documentation through appropriate interaction with physicians, advocating for appropriate reimbursement. Responsible for reviewing medical records to identify payer population as documented upon admission and throughout the hospitalization. Analyzes status of patient, current treatment plan, past medical history and identified possible gaps in physician documentation. Supports the accuracy and completeness of the clinical information used for measuring and reporting physician and hospital outcomes to reflect the patient’s true severity of illness, intensity of care, and risk of mortality. Qualifications Experience/Training/Experience: Associate's degree from an accredited College or University in the field of nursing required. Bachelor's degree from an accredited College or University in the field of nursing preferred. Minimum of five (5) years recent clinical experience in acute care setting (Critical care, Medical/Surgical or Emergency Medicine) required. Critical Care or Medical/Surgical preferred. Minimum of three (2) years recent experience performing CDI reviews in an inpatient acute care setting required. Minimum of 3+ years strongly preferred. Certifications/Licenses: Current license as a Registered Nurse in the State of California required. Current certification(s) for Certified Clinical Documentation Specialist (CCDS) or Certified Clinical Improvement Practitioner (CDIP) requried. Additional licensure/certification of 1 or more of the following credentials preferred: CCDS-O, CCS, RHIT. Other Skills and Abilities: Demonstrates knowledge and ensures compliance with The Joint Commission and Title 22 standards and guidelines. Demonstrates compliance with Healthcare policies and procedures at all times. Working knowledge of current ICD-10-CM coding guidelines, with working knowledge of Medicare reimbursement system required. Ability to set priorities and appropriately organize workload and complete assignments in a timely manner. Familiarity with basic medical laws and ethics, i.e. consents, confidentiality. Knowledge of emergency procedures, medical terminology and medical abbreviations. Demonstrates ability to relate to clinical personnel and medical staff, as well as ability to interact well with the public. Proficiency in working with word processing, databases and spreadsheets. Environmental: The incumbent may be exposed to infectious disease for prolonged periods of workday, chemical agents, dust, fumes, gases, extremes in temperature or humidity, hazardous of moving equipment, unprotected heights, and loud noises occasionally. Responsibilities ESSENTIAL FUNCTIONS Functions under the supervision of an orthopedic physician in accordance with established policies and procedures. Interviews, assesses, and evaluates patients of all ages to determine their physical condition and type of medical services needed, and orders initial and post-operative diagnostic studies appropriate for the patient’s medical condition. Accurately records and reports facts, including evaluation and treatment of the whole care of the patient and allows all entries into the patient chart to be reviewed and signed by the supervising physician. Adheres to medical regimens to stabilize chronically ill persons; adjusts regimen within established protocols. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional, or mental problems. Consults and collaborates with other health care professionals at Tuba City to established health care plans and to review patient progress and response to therapy, including the use of written referrals to other clinic staff, Community Health Nurses, specialty clinics, schools, etc. Maintains reporting and record systems related to patient care and progress, and as required by TCRHCC and Navajo Area Indian Health Service (NAIHS). Teaches patients and family member’s ways to maintain or improve their health status. Carries out treatments and emergency or other procedures required; administers medication; observes and documents patient conditions, changes and reactions to medications and medical treatment. Supervises orthopedic patients involving the whole management of care requiring the application of principles based upon the biological, physical, and social sciences. Evaluates health care plans and has ability to recognize the need for assessment by the physician and other health professionals. Evaluates total health care needs of patients and develops plans to meet these needs. Administration of joint injections, trigger point injections, punch biopsy, stapling, wound closure and maintenance, casting and splinting, local and digital blocks, suturing, and assess for medication management of chronic and acute disease states. Teaches joint replacement classes and completes health history assessments for surgical procedures. Assists in the surgical suite when accompanied by the surgeon. Works on a regularly assigned, rotational or call back basis to provide continuity of medical services during evenings, nights, holidays, and weekends as needed per interdepartmental agreement. Responsible for electronic health records data entry pertinent to patient service role. Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates) Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction. Completes all patient care documentation in the electronic health record entries accurately and in real-time. Participates in departmental workflow and or testing teams as related to electronic health record or other project initiatives. Performs other duties as assigned.
