Public Health Registered Nurse Case Manager - Well Child/Immunization

Position Summary: The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care, including well-child checkups, immunizations, tuberculosis and other infectious disease screening and diagnostic testing, management of chronic health conditions, and infectious disease case investigations. The role entails training Community Health Aides and Practitioners (CHA/Ps) to competently administer public health testing, examinations, immunizations, and assist with screening, diagnostic testing, treatment, and case investigation as needed. Responsibilities of the position includes participating in the design, execution, monitoring, quality assurance, and assessment of the Well Child/Immunization and other public health programs.
