JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Responsibilities include but are not limited to ensuring timely and accurate completion of assigned UAS assessments, LHCSA assessments, quarterly aide supervisions and all associated documentation. Must be a Licensed Registered Nurse in New York State, and have a minimum of 1 year of nursing experience preferably in the Assisted Living, Long Term Care or Geriatric fields. Responsible for quarterly aide supervisions as assigned. -Participate in chart audit/peer review process as needed. -Be in contact with facility administration ahead of all scheduled assessments to ensure all proper paperwork, materials and information are prepared and available. -Keep up to date knowledge base in reference to current admission assignments. -Be flexible and adaptive in performing ALP assessments. -Maintain client records in accordance with agency policy and procedure. -Observe, assess and report any adverse sign(s) and/or symptom(s). -Maintain strict client confidentiality. -Case conference with clinical staff and other departments as needed. -Obtain additional certifications and participate in additional training as requested by Director, which may include OASIS training, PRI/Screen Certification, and/or training in use of a QAS analyzer. -Other roles as assigned by DON on a particular day, which may include covering the lead RN role in an assigned home.
