This would be 5 days a week being 4 hour shifts. A caregiver who excels in this role will: Perform basic patient care activities. Assistwith procedures ordered by physician, with supervision from an RN, PT,OT or ST as appropriate. Assist patients with tending to personal care, activities of daily living and transfers/transport. Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff. Assist with keeping the patient's environment clean and orderly. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Escalate change in patient condition to patient’s physician as appropriate.
