Home Infusion Nurse Coordinator

The Home Infusion Nursing Coordinator is the clinical lead for the Home Infusion Nursing Team, who provides and utilizes clinical expertise and skills to coordinate and provide patient care, patient education and staff training regarding therapy and supplies, as well as clarifying competency and expectations of care in the home. The position will include direct home infusion patient care as needed. As the clinical lead for the YNHH Home Infusion Therapy team, acts as a nursing team resource and provides direct patient visits as needed. Responsible for the delivery of high-quality care as it relates to delivery of infusion therapy in the home or alternate setting. Facilitates with staff and provides direct to assigned patient and/or caregiver education on proper handling, storage and administration of infusion medications as well as signs and symptoms of access device complications. 5. In collaboration with pharmacy and nursing leadership, plays a significant role in the development, evaluation and implementation of standards of practice and related policies and procedures, ensures compliance with regulatory and accreditation standards relating to nursing. 6. In collaboration with pharmacy and nursing leadership, reviews, escalates and responds to patient and family complaints, investigates unusual events and ensures proper follow-up and documentation. 7.
