Home Health Certified Nursing Aide CNA

Pathwell Health Bridgeport, CT

The CNA/HHA will provide routine individualized healthcare based on a care plan set by the LPN to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient. We are currently looking to fill part-time and full-time positions. Home Health Weekend Registered Nurse

The RN would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The RN would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required.
