JOB PURPOSE: Conducting initial assessments and reassessments for members/clients for the elderly and disabled waiver program. KEY RESPONSIBILITIES: 1. Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs. 2. Requests medical and hospital records as needed to assist with determining eligibility. 7. Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status. 8. Attends and participates in mandatory in-services. 9. Complies with corporate compliance program. 10. Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary. 11.
