The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Provide skilled nursing care under the direct supervision of the Clinical Supervisor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the healthcare team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patient's conditions. Job Requirements The Home Training PD Registered Nurse evaluates and trains the dialysis facility’s home peritoneal dialysis patients, care partners and families for the home dialysis program. This position exercises knowledge of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment. Position must demonstrate familiarity with peritoneal dialysis equipment, procedures and the knowledge, skills, and judgement necessary to assume the responsibility of caring for the dialysis patient. Work Experience Registered Professional Nurse with current state licensure. Twelve (12) months of experience in nursing. Meets all applicable state/licensing requirements. Additional three (3) months of experience in the specific modality unless otherwise specified in specific state regulations. Documented training and experience in peritoneal dialysis and/or home hemodialysis and maintenance of the dialysis access. Basic training certificates if less than one (1) year in this modality. Previous supervisory and teaching experience preferred. Basic Life Support (BLS) certification. Current CPR Certification Demonstrated ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
