Job Overview Fairview Range is hiring a Registered Nurse (RN) Case Manager to join our Home Care and Hospice Team in Hibbing, MN. The RN Case Manager in Home Care and Hospice plays a critical role in coordinating and delivering high-quality, patient-centered care to individuals receiving services in their homes or in hospice settings. This position is responsible for managing a caseload of patients, developing individualized care plans, overseeing implementation of clinical services, and ensuring that care is provided in accordance with medical standards, agency policies, and regulatory requirements. Serving as the primary point of contact for patients, families, and interdisciplinary care teams, the Case Manager facilitates communication, monitors patient progress, and adjusts care plans as needed to ensure comfort, dignity, and optimal outcomes. Compassion, clinical expertise, and strong organizational skills are essential in this role. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach. An individualized plan of care with specific goals is developed for each patient in collaboration with the physician. Schedule: This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the day shifts with rotating weekends and on call. The position works approximately 10 weekends per year, and 2-3 days of call each month. Qualifications Certified Nursing Assisstant C.N.A. qualifications include: High school diploma or general education degree (GED). Current Pennsylvania State Nurse Aide Certification; Must be listed on state nurse aide registry and without allegations. At least (1) year experience with the frail or elderly.
