Overview Compassionate Hospice Care. Supporting Patients. Comforting Families. We are looking for a compassionate Hospice Licensed Practical Nurse (LPN) to join our team. Reporting directly to the Administrator or Executive Director, you will play a vital role in delivering hands-on hospice care to patients and families in their homes, assisted living, or facility settings. As a Hospice LPN, You Will: Provide direct nursing care services to hospice patients, supporting both patients and their families. Monitor patients actively and document any changes in condition, reporting as necessary. Administer medications and treatments according to the plan of care. Participate in infection control practices and standard precautions. Support bereavement activities and closure contacts with families as assigned. Participate in on-call duties and respond to patient needs during scheduled visits or emergencies. May assist with death pronouncements per state law and under RN supervision. Collaborate closely with hospice aides and other team members to ensure comprehensive care delivery. Compensation may vary within the wage range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Position Summary The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care. PRN hours M-F days to include some weekend availability if needed. Position located in the Gainesville, FL area and surrounding area Essential Functions Provide medical expertise in pain and symptom management, including terminal prognosis evaluation. Act as attending provider in hospice contract beds in collaboration with the hospice physician. Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses. Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes. Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings. Perform physical exams, diagnostic testing, treatment, follow-up, and consultations. Prescribe and administer medications and other therapeutic interventions. Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team. Support patients and families in clarifying care goals and end-of-life decisions. Maintain accurate and complete medical records to ensure compliance and reimbursement. Ensure informed consent documentation, including CPR and DNR discussions. Participate in interdisciplinary team meetings and on-call rotations. Educate internal staff and community members on palliative and hospice care. Shift Schedule: PRN Responsibilities Provides assistance in activities of daily living and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in OR room turnover. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Requires skills and abilities typically attained with 1 year of medical clerical experience; preferably in a hospital or physician office setting, or prior experience in customer service. Must possess excellent organizational skills, as well as effective human relations and communication skills.
