JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Other duties and responsibilities as assigned JOB REQUIREMENTS FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse has knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas. 2. Admissions Registered Nurse has the ability to communicate effectively with patients and families, physicians, referral sources and colleagues. 3. Admissions Registered Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge. 4. Admissions Registered Nurse has the ability to use the ANCHOR HEALTH information system and mobile technology platforms. 5. Admissions Registered Nurse has reliable means of transportation. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Position Summary The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners. The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process. Qualifications & Skills LVN, RN, or MSW required 1–2 years of healthcare marketing/sales experience preferred Minimum of 2 years hospice experience highly preferred Strong familiarity with local referral partners including: Hospitals Physicians Board & Care (B&C) Assisted Living Facilities (ALF) Residential Care Facilities for the Elderly (RCFE) Skilled Nursing Facilities (SNF) Excellent written and verbal communication skills Self-directed with the ability to work independently Determined, collaborative, and strong work ethic Consistent, persistent, patient, and compassionate interpersonal approach Valid driver’s license, clean driving record, and current auto insurance required Key Responsibilities Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners Plan and coordinate marketing events, educational sessions, and community luncheons Educate healthcare stakeholders on hospice and palliative care services Support patients, families, and referral partners throughout the referral and admission process Provide hospice informational / goals of care conversations with patients and families Facilitate initial patient consent process Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership Compensation & Benefits We offer a competitive salary and lucrative bonus structure: Salaried position with performance-based bonus potential Starting salary at $100,000+, based on experience Medical, Dental, Vision, and Chiropractic insurance 401(k) with company matching If you are passionate about hospice care, relationship-building, and making a meaningful impact in your community, we would love to hear from you.

The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. PRIMARY FUNCTION The Admissions Registered Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the admission of appropriate patients. JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Other duties and responsibilities as assigned JOB REQUIREMENTS FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse has knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas. 2. Admissions Registered Nurse has the ability to communicate effectively with patients and families, physicians, referral sources and colleagues. 3. Admissions Registered Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge. 4. Admissions Registered Nurse has the ability to use the ANCHOR HEALTH information system and mobile technology platforms. 5. Admissions Registered Nurse has reliable means of transportation. In the event the location does not have an Admissions Manager, the Admissions Registered Nurse will receive direction from the Patient Care Administrator or program designee QUALIFICATIONS FOR THE ADMISSIONS REGISTERED NURSE 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
