HOW YOU'LL WORK: You'll perform personal care needs for your patient and attend to their requests promptly. You will effectively communicate the patient's condition and significant changes to the assigned supervisory nurse and inform the caregiver of any issues. You'll be responsible for all practices and duties within the Hospice Aide scope of practice as outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Patient Care: Provide care that includes baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Assist in dressing and undressing patients as assigned. Assist in meal prep and feeding patients as assigned. Extended Services: Assist in ambulation and exercise as instructed by the primary care nurse or therapist. Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency. Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the primary nurse. Minimize Patient Risk: Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.). Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Policies: Adhere to the organization's documentation and care procedures and standards of personal and professional conduct. Operations: Attend in-service training and mandatory agency meetings. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude. Understanding of the hospice philosophy, comfortable providing specialized care to the terminally ill and secure with the issues of death/dying. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS : Meets the training requirements of the State and has successfully completed a state established or other training program that meets the State’s requirements of a competency evaluation program, or state licensure program. 1 year of experience is preferred, preferably in Hospice care. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will provide skilled interventions and instructions to the patient and caregivers to provide optimum symptom control and patient comfort. You will function as a liaison between disciplines during the admission process. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Ohio Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Ohio 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 5. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Qualifications Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director. Medicare home health or hospice experience is preferred.
