As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off. ON CALL 7 Days ON / 7 Days OFF Territory - Columbus and surrounding counties Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Admissions and On-call experience preferred Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

This position assists in responding to identified opportunities for improvement in the administration of hospice operations, program growth and financial performance. If you meet these qualifications, we want to meet you! Bachelor's Degree in a related field, or equivalent education and experience. Ability to become and maintain Hospice and Palliative Care Administrator (CHPCA) certification within 12 months from date of hire. Previous executive management experience preferred. Ability to conduct overnight and frequent travel. Familiar with Electronic Medical Record (EMR) systems. Knowledgeable about the Hospice Medicare regulations and Joint Commission standards. Family Home Health Network , is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area. We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change. THIS POSITION IS EVERY WEEKEND SATURDAY AND SUNDAY 7AM TO 7PM. Qualifications: Must possess current Illinois RN licensure Possess a minimum of two (2) years experience (Hospice Preferred) Current CPR Valid IL driver’s license and auto insurance policy HCHB EMR experience a plus!
