Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Admissions Registered Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge. 4. Admissions Registered Nurse has the ability to use the ANCHOR HEALTH information system and mobile technology platforms. 5. Admissions Registered Nurse has reliable means of transportation. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance. Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance. Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions. Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards. Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient. Help patients and families understand and navigate topics such as: Medication administration Hospice philosophy and services Symptom and pain management End-of-life processes and expectations Essential Functions Job Responsibilities Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
