Key Responsibilities: Comprehensive Patient Assessment: Collects and analyzes data on physical, psychosocial, environmental, self-care, educational, and discharge planning needs, responding to assessment calls when required. Treatment Plan Development: Develops a comprehensive care outline, updates master treatment plans, special programs, and problem sheets, collaborating with the treatment team to set goals and interventions. Effective Discharge Planning: Establishes individualized discharge plans tailored to the needs of patients and significant others. Crisis Management: Follows correct protocols for crisis management, demonstrating skills in defusing potentially combative behaviors and distinguishing between psychotic behavior and self-control problems.
