About The Role We are seeking a Nurse Practitioner (FNP) or Physician Assistant (PA‑C) with acute care experience in Emergency Medicine, Urgent Care, or Internal Medicine to join our growing team. As a field‑based Advanced Practice Provider, you will independently deliver comprehensive care in patients’ homes—conducting assessments, diagnosing conditions, managing treatment plans, and coordinating follow‑up care. You will be fully equipped to treat everything from common illnesses to complex injuries, while providing compassionate, whole‑person care across all age groups. The ideal candidate uses strong critical‑thinking skills, follows evidence‑based standards of practice, proactively identifies and solves problems, adheres to all clinical and safety standards, protocols, and performance metrics, and consistently provides care with compassion, empathy, and cultural competency. What We Offer Flexible 10‑hour shifts (market‑based) 4 shifts per week, including 4 weekend days per month, providing a balance between weekday and weekend scheduling.
