Hospice Nurse Practitioner

Certified Registered Nurse Practitioner (CRNP) – Hospice Primarily In‑Office | Regional Team | No Nights or Weekends Serenity Hospice is seeking a compassionate and experienced Certified Registered Nurse Practitioner (CRNP) to join our dedicated regional hospice team. This is a primarily in‑office position focused on meaningful, face‑to‑face clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams. In this role, you will play a vital part in ensuring exceptional end‑of‑life care by conducting required hospice face‑to‑face evaluations and supporting high‑quality, compliant, and compassionate patient care across our service region. Position Highlights Mostly in‑office role Face‑to‑face patient evaluations are the primary focus Regional team support across care centers No nights, weekends, or on‑call requirements Consistent schedule that supports work–life balance Key Responsibilities Conduct face‑to‑face hospice patient visits to evaluate continued eligibility in accordance with CMS Hospice Eligibility Guidelines Collaborate with the Hospice Medical Director and attending physicians to review patient status and adjust care plans as appropriate Provide clinical leadership and guidance to the interdisciplinary hospice team Support symptom management strategies and clinical decision‑making Authorize medications and treatments in accordance with state regulations and patient care needs Ensure accurate, timely, and compliant clinical documentation Participate in Interdisciplinary Group (IDG) meetings and care planning discussions Work primarily from an assigned Serenity Hospice office, with occasional local travel to patient homes or care facilities Qualifications Current and active CRNP license in Pennsylvania Hospice or palliative care experience required Thorough understanding of CMS Hospice Eligibility Guidelines Strong interpersonal, organizational, and documentation skills Ability to work collaboratively within an interdisciplinary team Willingness to travel locally within the region, as needed What We Offer No nights or weekends Flexible scheduling (Full‑Time or Part‑Time; minimum 25 hours/week) Competitive hourly rate: $60–$65 per hour Comprehensive benefits package, including: 401(k) Health, dental, and vision insurance Paid time off Supportive leadership and collaborative team environment Opportunities for professional development and continuing education The opportunity to make a meaningful difference in patients’ and families’ lives Locations & Regional Coverage Serenity Hospice operates four Care Centers in Pennsylvania .
