This role provides hands on care and support services for our senior age patients at our spacious care center in Bloomington, MN. University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care - Heart Failure team at the University of Minnesota Medical Center and the Clinics and Surgery Center (CSC) in Minneapolis. The position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services. The NP/PA performs independent follow-up of patients in the clinic setting. Participates in multidisciplinary advanced heart failure rounds in the inpatient setting, perform and documents inpatient assessments and formulates a plan to address complex, acute, and chronic health needs to advance the plan of care. Includes ordering appropriate tests, labs, and interpretation of results.
