Core Duties: Compiles data for the purpose of quality assurance and compliance with organizational and department policies, work aids, and algorithms Holds other Patient Logistics staff accountable to the established standards Accurate and timely distribution of daily reporting Participate in Patient Logistics growth and improvement Support program director and manager in preparation of reports, program material, research, data collecting and analysis for various meetings and or programs Manage Transfer Center calls in accordance with the established workaids and algorithms and support staff in learning and upholding these work processes Provide supervisory support to Patient Logistics RN and other staff as needed Ability to function and lead independently with self-initiative and discipline Other duties as assigned to meet the program and corporate needs Collect objective/subjective data for effective and appropriate transfers of patients Determine facility designation based on diagnoses, planned procedure and level of care requirements Maintain constant communication with both internal and external sources of information concerning patient transfers Utilize nursing knowledge and assessment to prioritize patient transfers based on urgency of patient’s condition, utilization of available beds. Communicate effectively with hospital staff to allow for patient flow. Responds efficiently during emergency situation for appropriate transfer of all patients Evaluate effectiveness of patient transfer in collaboration with Dignity Health facility Make judgments, decisions and modify patient transfer priorities based on evaluation Perform legible, timely, concise and accurate documentation according to policies and procedures Proficient and competent with computerized documentation Accept responsibility and accountability for own decisions and behaviors Recognize , accept and cooperate with direction from facility leadership Participate in orientation of new staff Serve as resource to staff from other hospitals and vendors Continue education by attending non-mandatory education offerings specific to area of practice Maintain awareness of issues related to nursing profession Provide measurable written goals appropriate to level of experience during annual appraisal process Demonstrate effort to achieve pre-set goals throughout the year Maintain a safe working environment by utilizing appropriate resources, protocols, procedures, and communication to appropriate personnel Apply safety precautions and principles to patient care, including all National Patient Safety Goals Communicate clearly and effectively using proper communication etiquette Use medical terminology accurately Include reporting abnormalities or change in patient condition to appropriate personnel in a timely manner Participate in service line growth and Dignity Health quality initiatives Include patient satisfaction and core measures Job Requirements Minimum Qualifications: Current RN license Proven proficiency with all Microsoft Office applications Familiarity with TeleTracking, Cerner and Midas Excellent verbal and electronic communication stills, including professional telephone and email etiquette RN with 3-years of clinical experience, one to include acute hospital Must have the ability to communicate effectively with a variety of customers including patients, providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers Ability to work independently and apply critical thinking skills toward making appropriate decisions, as well as in a team environment Ability to multi-task and prioritize Detail oriented Excellent organizational and communication skills and ability to meet timeframes Ability to learn quickly and retain complex information Travel as needed Ability to lift a minimum of 20 pounds Preferred Qualifications: Progressive responsibilities leading up to or including supervisory role Experience in Utilization Management, Case Management or Care Coordination, Managed Care is preferred Where You'll Work Inspired by faith. The Care Giver/Med Tech role includes providing hands on care, physical and emotional support to each resident while maintaining a safe and comfortable home like environment. Responsibilities: Care Giver: Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift. Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities. Responds to security system and resident call bells promptly. Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs. Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms. Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite. Maintains cleanliness of resident’s room and work areas. Practices good standard care precautions of cleanliness, hygiene and health. Helps residents maintain independence, promotes dignity and physical safety of each resident. Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed. Engages residents in life skills and other life enrichment activities. Greets and assists all internal and external customers, guests, family members, residents, vendors and team members. Does resident laundry as assigned and needed. Medication Tech duties: Reviews service plan to learn pertinent information about residents. Assists/observes medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass. Documents and initials form as medications are given including appropriate documentation for refusal or missed doses. Maintains confidentiality of all resident information including resident medication. Reports all resident concerns made while assisting resident with the medication to the Nurse and/or Health Care Coordinator (HCC). Restocks medication cart after all medication passes. Assists in checking medication regardless of packaging system. Counts all narcotics with another Medication Care Manager or Nurse each shift. Maintains and cleans the Medication Room, med charts, treatment carts for neatness cleanliness, availability of medications and expired medications. Follows re-fill process for medications. Participates in the development of the Service Plan and monthly updates. Takes and records temperature, blood pressure, weight, pulse and respiration rates. Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately. Other duties as assigned by the Supervisor. Qualifications: Certified Nurse’s Aid certification preferred. High School diploma/GED Must be 18 years of age. CPR Certification preferred First Aid Certification preferred Previous experience working with seniors preferred.
