Position Summary The RN Home Admissions facilitates timely access to hospice care by providing comprehensive preadmission and admission services. This role collaborates with internal staff, community partners, and referral sources to overcome barriers to admission and ensures patient, and family needs are met with compassion and professionalism. The RN conducts initial assessments, develops immediate care plans, coordinates care transitions, and educates stakeholders on hospice philosophy and eligibility criteria. Educate patients, families, and referral sources about hospice services and eligibility criteria. Conduct initial assessments, including physical, functional, psychosocial, and spiritual needs. Apply appropriate nursing interventions and document care accurately in the electronic medical record. Determine hospice eligibility in accordance with Medicare Local Coverage Determinations and state regulations. Coordinate physician communication, obtain admission orders, and initiate medications and DME. Develop the initial plan of care and facilitate seamless patient transitions. Provide emotional support to patients and families during the admission process. Maintain HIPAA compliance and assess safety and risk factors per organizational policies. Qualifications Knowledge and Critical Skills Knowledge of hospice services, philosophy, and applicable admission regulations. Understanding of Advance Directives, Medicaid processes, and healthcare placement options. Skilled in pain and symptom control, customer service, and interdisciplinary collaboration. Ability to identify and address psychosocial, cultural, and spiritual needs. Strong verbal and written communication; capable of managing sensitive conversations. Demonstrated clinical judgment, critical thinking, and problem-solving. Adaptable to change, able to work autonomously, and manage multiple priorities. Competence with electronic medical records and mobile technology. Experience Minimum two years of experience in a home, hospital or long-term care setting required. License / Certification Current Registered Nurse license in the State of Florida. Will consider LPN. Current CPR certification. Valid driver’s license and acceptable driving record. Level II AHCA Background Screening. Certification in Hospice and Palliative Nursing (preferred). Education Diploma in Nursing, Associate Degree in Nursing (ADN), or Bachelor of Science in Nursing (BSN) from an accredited program. Other Requirements Medical clearance for communicable diseases. Compliance with OSHA and CMS vaccination requirements, TB, Hepatitis, and Flu. Reliable transportation and ability to work flexible hours and locations. Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. ACLS from the American Heart Association is required within six (6) months of hire or transfer for Progressive Registered Nurses in Intensive Care Units. PALS from the American Heart Association is required within six (6) months of hire or transfer for the following departments: PICU (660401) and Trauma Center (713100). Fellowship RN's must obtain their ACLS, PALS and NRP within six (6) months of hire or transfer if required by the department. Crisis Prevention Institute training within ninety (90) days, and then annually, is required for Psych Unit. Preferred Licensure/Certifications: Professional certification preferred. Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. Overview Job Duties An Advanced Practice Provider V (APP V) is a direct patient care provider licensed in the State of Florida and/or Georgia and provides medical services to patients as defined by their scope of practice and under the supervision of a physician licensed to practice medicine in the State of Florida/Georgia. Clinical Area - 90% of their time is spent with inpatients, 85% of this time is in the OR. Responsibilities Essential Functions • Endoscopic saphenous vein harvesting GSV, open vein harvest, GSV and LSV, radial artery harvest, open and endoscopically, open femoral artery/vein cannulation for bypass. • Invasive monitoring and placement of arterial and central venous lines, insertion and removal of thoracostomy tubes, insertion of central lines, including Swan-Ganz catheters, TLC, insertion and removal of IABP, removal of temporary pacer wires, insertion of arterial lines, NG/OG tube insertion, insertion of foley catheters, 1st assistant techniques. • Assesses patient health status including state of wellness, compliance with care plan and determines appropriate diagnostic and therapeutic procedures. May perform those routine medical treatments and diagnostic procedures for which they are qualified to perform by training and which fall within the normal scope of practice of the supervising physician. • Performance of histories and physical exams, pre and postoperative patient care, critical care, daily progress notes, obtain surgical consent, patient education, and discharge summaries. • Patient care in ICU including advanced knowledge of hemodynamic management, ventilators, and artificial cardiac devices. • First assist in surgery in all cardiac, thoracic and vascular procedures. Recognize serious, life-threatening emergencies, evaluate them appropriately and ensure patient receives prompt care to stabilize. Triages patient calls and evaluates patient problems. • Opening and closing of sternotomies, emergency opening of post op cardiac patient, emergency intubation, perform required would care under the direction of the CT Attending. • Educator and trainer for nurses, residents and fellows. • Participates in quality management and performance improvement activities. • Call coverage may be required specific to departmental needs.
