MDS Coordinator - Nursing Home Located in Antioch, TN Salary Up To $115K Per Year; * Based on experience * Responsibilities : Conduct and coordinate the development and completion of the resident assessment (MDS) Maintain and periodically update written policies and procedures that implement MDS and care plan. Assist the resident in completing the care plan portion of the resident’s discharge plan. Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents. Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings. Responsibilities may include: Providing calm and consistent supervision Offering appropriate redirection and reassurance Supporting established routines Monitoring for confusion, agitation, wandering or changes in behavior Encouraging safe and appropriate activities Maintaining a quiet, respectful and supportive environment Following client-specific communication and safety strategies Immediately reporting significant behavioral or cognitive changes Caregivers assigned to dementia or memory-care clients must remain patient, attentive and aware of the client’s location and safety throughout the entire shift. Mobility, Transfer and Higher-Assistance Support Some clients may have limited mobility, be at high risk for falls or require significant physical assistance. Depending on the care plan and caregiver training, responsibilities may include: Assisting with walking and balance Supporting wheelchair mobility Assisting with bed mobility and repositioning Helping clients move between the bed, wheelchair, chair, commode or toilet Using an approved gait belt when directed Assisting with sit-to-stand or stand-to-sit transfers Using a mechanical or Hoyer lift only after required training and authorization Following all one-person or two-person transfer requirements Requesting assistance when a transfer cannot be completed safely Caregivers must never attempt an unsafe transfer or use equipment they have not been trained and authorized to operate. Organization: Radiation Oncology Job Summary: The Advanced Practice Provider (APP) plays a vital role in delivering specialized care and managing the health needs of patients in either inpatient, ambulatory, or a combination of both settings. DEPARTMENT SUMMARY We provide multidisciplinary, state of the art therapy that utilizes techniques such as stereotactic radiosurgery, intensity modulated radiotherapy (IMRT), three dimensional treatment planning, brachytherapy, and intraoperative radiation therapy at five convenient locations. We are dedicated to improving treatment outcomes of all diseases treated with radiation and to finding new medical uses of radiation. Related to these goals is the education of graduate students, post-doctoral students and residents who will enter the field of radiation medicine including the allied field of Medical Physics. Our team provides state-of-the-art anti-cancer therapy for patients through the use of new technology and continuous review of patient treatment outcome. We will continue to evaluate and improve the therapeutic effects of radiation from five fronts: Basic science research to identify new biological modifiers and therapeutic targets. Interaction with industry (pharmaceutical, imaging and radiotherapy) to validate the efficacy of new therapeutic tools Medical physics research to improve the accuracy, precision and methods of radiotherapy delivery Clinical research to study the benefits of new therapy treatments in patients POSITION SHIFT Location: Lebanon 4 days in clinic, 1 administrative day No call, weekends or holidays KEY RESPONSIBILITIES Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care. Identify urgent or emergent reactions during infusion, initiate appropriate interventions, and coordinate emergency transfers when necessary.
