Description Kindred Hospital Denver and Acute Rehabilitation Unit (ARU) is an all-private room 68-bed hospital located on one of Denver’s important medical campuses, just east of St. Joseph’s Hospital and Presbyterian St. Luke’s Medical Center. With 55 long-term acute care hospital (LTACH) beds, we offer the same in depth care you would receive in a traditional hospital, but for an extended recovery period. Our 13-bed ARU is designed for people who have experienced the debilitating effects of an acute injury, impairment or illness and like the LTACH, we partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. Job Summary Under direct supervision of Registered Nurse, provides nursing care within the scope of licensure for assigned patients according to established nursing standards and hospital policies and procedures. Essential Functions Under direct supervision of a Registered Nurse administers medication, performs treatments and patient care responsibilities as prescribed. Consults with and keeps Registered Nurse informed of activities, symptoms, reactions, and changes in the patient’s condition with regard to assigned patients. Provides appropriate pain management techniques and educates the patient and family regarding pain management. Administers medication following the five rights of medication administration within scope of practice. Assists with the rehabilitation and treatment of patients in accordance with the medical plan of care, updates, and revises plan of care under the supervision of Registered Nurse. Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills as directed by the Registered Nurse. The LPN Medication Reconciliation Nurse will perform medication reconciliation of orders with our contracted facilities’ Medication Administration Records (MARs) and Electronic Medical Records (EMR) to ensure accuracy and avoid medication errors. They will coordinate the process for any corrections and ensure follow-up was completed. Complete monthly medication reconciliation of facility MARs with InnovAge medication list. Communicates medication-related issues to all necessary parties. Pulls necessary reports throughout the month to ensure accurate orders. Documents in EMR and other applications as needed.
