Responsibilities: Coordinates care of transplant patients through collaboration with inpatient and outpatient multidisciplinary teams, pharmacies, community providers while following regulatory guidelines set forth by regulatory agencies. Manages patient medications per policies and procedures. Triages patient issues and facilitates coordination of patient care as directed by providers or per protocol. Educates patients along the continuum to assist understanding of processes and procedures surrounding transplant care.
