Performs nurse-midwifery duties on a regular 24 hours basis which includes; conducting prenatal and postnatal office visits and family planning/GYN, attending patients in labor and at delivery, consulting with attending physician when necessary, making daily rounds to nurse-midwifery patients. 2. Position Summary: The Stroke Coordinator is responsible for supporting the activities focused on improving and reporting stroke care outcomes.Provides outcomes reports and identifies performance and process improvement opportunities.Provides stroke education to staff and community and participates in stroke center research. The Advanced Practice Provider (APP) will primarily support the Women’s Health/Gynecologic Oncology outpatient practice at William Beaumont University Hospital in Royal Oak. We are seeking a highly skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our Cardiothoracic Critical Care Team within the Cardiovascular Surgery Service Line. This role provides comprehensive patient care from admission through discharge, with a focus on perioperative management in the Cardiac Surgical ICU. The APP will manage ventilators, sedation, hemodynamics, and diagnostics, and coordinate care with multidisciplinary teams. Clinical expertise in managing complex conditions such as shock, sepsis, respiratory failure, heart and renal failure, arrhythmias, and post-operative cardiac patients is essential. Responsibilities may include advanced procedures such as central and arterial line placement, pulmonary artery catheter placement, and point-of-care ultrasound. Additionally, the APP may sit on committees and participate in Quality and Process Improvement Initiatives. This is a 24/7 service requiring rotation through days, nights, weekends, and holidays, with regional cross-coverage at other hospital sites. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan.
