Title: Nurse Practitioner, Advanced Practice Provider Company: Oak Street Health Role Description: The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on medicare to keep them happy, healthy, and out of the hospital. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Specialized care provided in the CICU includes cerebral oximetry monitoring, inhaled nitric oxide, temporary pacemakers, heart failure management, open chest procedures, and continuous renal replacement therapy (CRRT). Extracorporeal life support represents a highly specialized form of cardiopulmonary support that includes the use of ECMO and ventricular assist devices (VAD). The SANE trainer is a central member of the SANE medical team and training at Harborview Abuse and Trauma Center (HATC). Required Experience: Minimum of 2 years of consecutive experience as a Nurse Practitioner specializing in women's health and treating chronic conditions such as PCOS, Endometriosis, Hypothyroidism, Hashimoto’s, Metabolic Syndrome, Menopause, Fertility, Postpartum, PMDD, and Obesity. Independence: Ability to operate effectively as an independent provider with strong diagnostic and problem-solving abilities. Communication Skills: Exceptional written and verbal communication with a focus on clarity and compassion. Webside Manner: Strong ability to connect with patients virtually and provide empathetic care. Commitment to Evidence-Based Care: Dedication to delivering treatments grounded in the latest research. Technical Proficiency: Highly skilled in navigating multiple computer screens and proficient in tools such as EMR, text expanders, Gmail, Google Calendar, Zoom, and scheduling platforms.
