Measures and records food and liquid intake and output and checks and records vital signs. Provides patient help with walking, exercising, and moving in and out of bed. Performs minor physical therapy routines and turning in bed to prevent bed sores. May collect urine samples, attach EKG leads and remove catheters. Assembles, monitors, and discontinues feeding of intravenous nutrition. Assists lower-level certified nursing assistants. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Shifts: 6AM-9AM 4PM-7PM Overview The Certified Medication Technician (CMT) assists residents with medications as allowed under State guidelines and is responsible for safe administration of prescribed medications and treatments. The CMT is responsible for the safe administration of prescribed medications and treatments as listed on Medication Administration Record (MAR). The CMT meticulously correct records of care including Medication Observation Record. The CMT provides personal care and personal care to residents as needed/assigned and ensures delivery of care and services to residents in the community to include Activities of Daily Living (ADLs), room cleanliness, scheduled activities and dining services and individual service plan need of each assigned resident. Qualifications: The CMT must be in good standing and meet all applicable federal and state certification requirements. Completion of high school or GED equivalent. Must be able to read, write and follow oral and written communication Must speak and understand English Positive attitude toward those diagnosed with mental illness as well as the elderly. Emotional maturity and sensitive to the needs of the mentally ill. Certification in CPR and first aid preferred or as required by the state. Must be neat and organized. Must be computer literate. Must have a positive Criminal Background Screening. Notify RCD or Executive Director (ED) when supply is less than two days and medicine has not been received. Receive and verify medicine received is accurate according to pharmacy label and MAR. Notify RCD of discrepancies. Store medications properly and ensure lotions, ointments, and liquids stored separately from oral medications. Inspect medications weekly to ensure expired medications are removed. Verify current MARs are compared to new MARs at change over time and notify RCD of discrepancies. Remove discontinued medication from medication cart and give to RCD. Provide personal care to residents when needed, including, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, eating, etc. Provide general ADL care including, but not limited to vital signs, temperature, blood pressure, and weight. Document appropriate information in daily log as required, notify Shift Supervisor (SS) and RCD of concerns. Answer calls and assist residents with needs as assigned. Be the Best Hospice IPC RN You Can Be If you meet these qualifications, we would love to meet you: Must be a licensed Registered Nurse in the state of practice; associate degree preferred; CPR certification required in Georgia and California Requires previous experience in hospice, home health, medical-surgical, or population-specific care (e.g., geriatrics, pediatrics); one year of experience required in Connecticut Willingness to serve on-call and uphold the organization’s values and mission as a role model for compassionate care Must have a valid driver’s license, reliable vehicle, and meet minimum state-required liability auto insurance Physical demands include frequent lifting (up to 200 lbs with assistance), walking, standing, sitting, stooping, and driving Must be able to work in varied conditions, including exposure to moderate noise, chemicals, outdoor environments, and temperature changes Responsibilities: Provide comprehensive nursing care by assessing, planning, implementing, and evaluating individualized care plans for hospice patients and their families Coordinate with physicians, the interdisciplinary team, and caregivers to ensure seamless, compassionate care and proper use of resources Maintain accurate and timely documentation, updating patient records throughout the shift to reflect current needs, goals, and interventions Educate and support patients and caregivers on medication use, personal care, mobility, and preparation for end-of-life Offer emotional and physical support at the time of death and participate in 24/7 care coverage alongside the hospice team Attend team meetings, assist with hospice aide supervision, participate in training and special projects, and adhere to agency and state/federal nursing regulations Our Investment in You Caring for others starts with caring for you.
