The RN - Triage Team provides immediate clinical support within the PACE center by prioritizing participant needs, managing urgent concerns, and coordinating care. This role ensures timely response to participant and staff inquiries, supports safe medication administration, and facilitates smooth clinic operations. Job Description Job Summary: The Clinical Nurse C-CVL (CNC-CVL), functions effectively in Magnet Recognized large tertiary academic medical center with level one trauma designation, ST-Elevation Myocardial Infarction (STEMI) receiving center designation and comprehensive stroke center accreditation. Demonstrates advanced knowledge in adult cardiovascular nursing and functions in a fast-paced adult cardiovascular lab. Follows the nursing process to perform simple and complex diagnostic and therapeutic invasive cardiovascular procedures for adult patients with cardiac dysfunction. The Registered Nurse Case Manager performs daily coordination of acute and post-acute care with facility staffing. The RN Case Manager actively assists providers and facility staff in managing InnovAge admitted participants by facilitating care through interaction with facility departments and community services. Reviews for medical necessity and level of care appropriateness in collaboration with the InnovAge IDT while coordinating post-facility discharge planning and support utilization review and improvement activities. The role aims to optimize positive health outcomes and prevent hospital readmissions by focusing on the transitional care period. Participant Nursing Care Coordination – 70% Assesses, develops, plans, and evaluates care provided to participants while admitted to hospital settings via facility EMR and discussions with facility staff. Closely monitors all patients at skilled status within SNFs, including short-stay and long-stay residents, working with the IDT to ensure that skilled status is only provided when necessary and for the minimum number of needed days. Coordinates transfer of patients to appropriate facilities; maintains and provides required documentation. Maintains and reviews participant records, charts, and other pertinent information.
