As an RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The Weekend RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Experience with IVs Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own on a day-to-day basis with activities that include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to be able to travel across the service area

Department Description The Adult Internal Medicine Unit cares for medicine patients ranging from young adults to the elderly presenting with a wide variety of diagnoses. Patients found on this unit are often admitted for acute exacerbations of complex chronic conditions with a focus on pulmonary, gastrointestinal, sickle cell, renal failure, medical-psychological, and endocrine conditions. In addition, patients are admitted for diagnostic work-up and treatment of new-onset infectious disease processes. Continuous video camera monitoring and cardiac telemetry are also available, if needed, to provide comprehensive, safe patient care. Collaboration between our physician colleagues (teaching services and attending services) and multidisciplinary teams contributes to the high functioning teamwork found on this unit. The unit is also supported by the Geriatric Clinical Nurse Specialist (CNS), ACE RN Program Coordinator, Medical/Surgical CNS/Educators and an Oncology CNS. This cohesive team approach allows our nurses to provide a high standard of innovative care to patients. Nursing based research that enhances nursing or improves patient care and safety is actively supported here. In addition, a nursing clinical leadership structure is provided to support and mentor nurses of all levels of experience. Job Summary: Independently manages the care of low-risk women during antepartum, intrapartum and postpartum periods. Co-manages higher risk patients when determined appropriate by CNM and MD. Provides standard maternity care and counseling to women, adhering to the standards of the American College of Nurse Midwives, procedures and instructions regulated by the State Nurse Practice Act and The Permanente Medical Group. Essential Responsibilities: ANTEPARTUM Evaluates patients general and obstetrical health status. Determines pregnancy status, duration of gestation, presentation, and position of fetus, fetal heart rate, and determines whether all statistics are within normal limits. Obtains an accurate medical, obstetrical, gynecological and social history and an account of the course of the present pregnancy. Observes signs of the members general and obstetrical health status. Performs all appropriate physical examinations appropriate to CNM care needed, including the pap smear, breast examination, and pelvic examination. Records all findings of examinations, histories and significant observations on patients chart. Consults with appropriate MD regarding any abnormal findings. Orders appropriate lab and medical imaging studies as needed. Institutes preventative measures and detects abnormal conditions. Obtains consultation and assistance from ancillary services as needed. Teaches, counsels, and guides individual members regarding their individual health care. Teaches antepartum classes as needed in individual facilities. Evaluates maternal general health status and well being. INTRAPARTUM Performs medical screening exam to diagnose labor. All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
