The Director of Telehealth is the single accountable owner of this program: responsible for building and managing the clinical team, redesigning the platform and workflows that support it, launching and overseeing CCM/BHI operations, and ensuring the program meets its coverage, quality, financial, and retention goals. This is a hands-on operational leadership role. The Director will directly supervise a rotating team of Nurse Practitioners/Physician Assistants (FTE and PRN) delivering after-hours and weekend telehealth coverage, chronic care management, and behavioral health integration services across CareMed's partnered skilled nursing, assisted living, and long-term care facilities. Key Responsibilities Program Build & Operations • Own end-to-end build-out and ongoing management of the telehealth program, including the clinical rotation schedule (weekday overnight and full weekend coverage). • Assess and restructure the current telehealth platform and clinical workflows to resolve existing fragmentation and inconsistent coverage. • Directly supervise the clinician team: scheduling, coaching, performance management, and clinical escalation support. • Serve as the primary liaison between the telehealth team, facility partners, and executive leadership on program performance. CCM & BHI Program Ownership • Own the Chronic Care Management (CCM) and Behavioral Health Integration (BHI) program build: patient eligibility screening, enrollment workflows, and patient consent. • Ensure documentation standards and billing accuracy for CCM (CPT 99490, 99439) and BHI (CPT 99484), including correct application of Medicare Part A stay exclusions and place-of-service rules. • Oversee accurate coding and documentation for after-hours telehealth encounters billed under Subsequent Nursing Facility Care codes (CPT 99307–99310). • Monitor CCM/BHI enrollment volume and encounter volume against revenue targets, and adjust outreach or staffing to close gaps. Quality, Compliance & Financial Accountability • Hold the team accountable to chart closure compliance (95%+ within 48 hours) and clinical audit/quality standards. • Track and report on avoidable hospitalization/ED reduction and 30-day readmission rates for the attributed patient population. • Own the program's financial performance: CCM/BHI and encounter revenue versus target, and the program's overall cost-to-revenue ratio. • Ensure all documentation and billing practices meet CMS and payer compliance standards; serve as the point of escalation for coding/compliance questions specific to telehealth, CCM, and BHI. Team Leadership & Retention • Own the retention strategy for the telehealth team: onboarding, workload distribution, escalation support, and culture. • Monitor and manage PRN utilization to ensure sustainable coverage without over-reliance on the backup pool. • Track team turnover and time-to-fill metrics, and proactively address drivers of attrition. Required Qualifications • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one of CareMed's operating states (OK, KY, WV, OH); multi-state licensure or eligibility strongly preferred. • Minimum 5 years of clinical experience in primary care, geriatrics, long-term care, or a related acute/post- acute setting. • Minimum 2–3 years in a clinical leadership, supervisory, or program management role overseeing other clinicians. • Direct experience with telehealth service delivery, including platform-based virtual care in skilled nursing, assisted living, or similar settings. • Working knowledge of CMS billing and documentation requirements for Chronic Care Management, Behavioral Health Integration, and Transitional Care Management. • Demonstrated ability to manage a rotating, multi-shift clinical schedule (including nights and weekends) across multiple states or regions. ESSENTIAL FUNCTIONS OF POSITION: • Responsible during the shift for the total nursing care of residents in the assigned unit. • Assumes responsibility for compliance with Federal, State, Local, and company regulations. • Explains procedures and treatments and care plan to resident. • Administers prescribed medications and treatments in accordance with approved nursing techniques. • Transcribes physician's orders. • Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. • Receives /gives report from/to nurse he /s he is relieving/coming on and records sufficient infonnation and implements appropriate follow-up action as necessary • Follows established procedures for charting and reporting all reports of incident/accidents for res idents and employees. • Conducts frequent rounds to evaluate effectiveness of care delivery. • Prepares equipment and a ids Physician during treatment and exami nation of resident. • Maintains awareness of comfort and safety needs ofr esident. • Observes resident, records significant conditions and reactions, and notifies supervisor or Physic ian of resident ' s condition and reaction to drugs, treatments, and significant incidents . • Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from nonnal and assess condition of resident. • Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. • Assists in completion of the care plans (acute and long term) and is involved in the care conferen ce as deemed necessary. • Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. • Ensures that documentation i·s reflective of the care plan and that the approaches on the care plan are being followed. • Assures that assigned CNAs are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. • Monitors that CNA documentation is completed daily . • Takes report from CNAs to facilitate accurate documentation in resident medical record. • Follows company policy and procedure in the event a restraint must be utilized. • Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. • Identifies residents at risk for nutritional problems and implements preventative measures. • Records new/changed diet orders. Forwards information to the Dietary Department. • Makes beds, bathes and feeds resident, when necessary. • Assures documentation is accurately completed for admission, discharge and transfer. • Supervises and evaluates implementation of the "Resident's Rights". • Participates in department or unit quality improvement activities. • Follows facility procedure requesting supplies and charges for items ordered through Central Supply. • Participates and attends periodic in-services and staff meetings. • Perform additional duties as assigned. EDUCATION/ REQUIREMENTS: Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required.
