Position Summary: To provide quality patient care on an ongoing basis. The Certified Nursing Assistant provides basic care to patients and assists with ADL's. Supports the hospital philosophy, goals and objectives. Is responsible for care for patients from birth to elderly. Position Qualifications: High school diploma or equivalent. The case manager facilitates the coordinated utilization of resources for maximization of health outcomes, patient/family satisfaction and financial outcomes. Position Qualifications: Minimum Education: Graduate from an LNL school for LPN's or LVN's. In addition, must be licensed as an LPN in the State of Alaska in good standing. Position Summary : This is a grant funded position. Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This at times does require village travel. Educator will be responsible for ordering labs for patients according to hospital policies. Educator will be responsible for documenting education in electronic health record. This position will assist in the planning, coordinating and providing of diabetes-related health promotion activities in the YK Delta Region. Patients and activities are accessed via multiple village trips per month. Educator will work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care. Educator assists Diabetes Coordinator with maintenance of the Diabetes Registry and grant requirements. Educator works closely with the Registered Dietitian Clinical Diabetes Educator and Diabetes Pharmacist to ensure comprehensive care of clients. Educator participates in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events. Educator participates in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders. Educator believes in and promotes Napartet and all corporate endeavors. Performs other duties as assigned or required. Position Qualifications : Must have completed a program of education in nursing from an NLN approved institution. Minimum Associate's Degree in Nursing required. BSN or MSN preferred. New graduates (with relevant license-see below) may be considered. Two (2) years experience in a clinical health setting preferred. Demonstrated experience collaborating with a variety of disciplines for project completion. Background in Diabetes Education preferred. Valid license in good standing in the State of Alaska as a Registered Professional Nurse. Must obtain valid CDCES (Certified Diabetes Care and Education Specialist) certification within 2 yrs. Preferred Basic Life Support (BLS) certification. Work in a typical office, hospital, and village clinic setting. Comprehensive knowledge of PC computers. Ability to operate diabetes care medical equipment such as glucometer, ht./weight scale, etc. Requires travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment, occasional long working hours. Position Summary: The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care, including well-child checkups, immunizations, tuberculosis and other infectious disease screening and diagnostic testing, management of chronic health conditions, and infectious disease case investigations. The role entails training Community Health Aides and Practitioners (CHA/Ps) to competently administer public health testing, examinations, immunizations, and assist with screening, diagnostic testing, treatment, and case investigation as needed. Responsibilities of the position includes participating in the design, execution, monitoring, quality assurance, and assessment of the Well Child/Immunization and other public health programs. This includes identifying areas of improvement and proposing effective strategies to enhance program performance. The incumbent is expected to travel to various villages to render services and provide on-site instruction, and supports CHA/Ps in conducting examinations, assisting with testing, and administering immunizations, ensuring adherence to health and safety protocols. The Case Manager provides education on topics related to immunizations, chronic care, preventive health examinations, and diagnostic and surveillance testing, and is required to communicate effectively with diverse audiences, ensuring the information presented is understood and applied appropriately. Additionally, the Case Manager assists in providing instruction during Health Aide Training and at Continuing Medical Education (CME) events for CHA/Ps and other clinical staff. Other duties include coordinating with other health care providers, maintaining accurate records of patient interactions, monitoring patient progress, and ensuring patients' health needs are met effectively. The Case Manager must stay updated with the latest public health knowledge and developments, applying this knowledge to improve patient outcomes and overall program effectiveness.
