In this hybrid position, the clinician will float to health centers to provide in clinic patient care within a designated region, cover telehealth sessions (working from a health center or administrative building), cover asynchronous e-visits, support health center teams by managing EHR Inbaskets and follow up, and assist as needed in the Care Coordination Team. The exact mix of these activities will depend on the needs of the organization. The clinician will float to various health centers and will work under the supervision of the Center Manager and in collaboration with the health center care team, to provide direct patient care in accordance with PPSNE protocols both in person and on telehealth. While working Care Coordination Team (CCT) sessions, under the supervision of the CCT Manager, the clinician is responsible for managing follow-up of abnormal results, signing off on lab reports and providing clinical and clinician support to the CCT. The clinician will communicate with patients regarding more complex care plans, such as beta quant HCG follow-up and urgent follow-up. \n Education and Experience Graduation from a recognized Nurse Practitioner, Physician Assistant or Nurse Midwife training program or Medical School. Appropriate professional certification (ANCC, NCC, ACNM, NCCPA or MD) Active licensure in good standing (State Medical License for CT and/or RI, Controlled Substance Registration, DEA) Basic Life Support certification Experience in sexual and reproductive healthcare Primary care experience preferred Bilingual preferred (English-Spanish) Key Qualifications Demonstrated leadership skills – ability to lead teams and desire to work as part of an interdependent team. Essential Functions Patient Care Ensures high quality direct patient interactions in person, on telehealth, and on asynchronous e-visits based on empathy for the sensitive issues facing our patients and an understanding of the diverse experiences of those seeking our services. Performs the physical exam, orders and/or performs appropriate laboratory testing and recognizes deviation from normal. Performs and interprets limited pelvic ultrasound Implements individualized regimen of care in compliance with PPSNE medical policies and procedures, including patient education and referral of services. Initiates appropriate tracking of abnormal results and referrals Completes documentation of patient care in the medical record in a timely and complete manner. Maintains appropriate billing and coding practices. Provides telephone triage for incoming clinical calls received by the call center, as needed. Communicates with patients regarding more complex care plans such as beta quant HCG follow-up, breast and cervical health follow-up, and any urgent follow-up. Completes follow up on laboratory and imaging results per PPFA Medical Standards and Guidelines (MS&G). Provides medication abortion and participates in the evaluation and management of patients receiving procedural abortion services. Maintains clinical quality assurance standards as set forth by PPSNE. Appropriately recognizes the boundaries of one’s own knowledge and utilizes the appropriate channels for support. Participates in providing all the medical services offered by PPSNE after appropriate training and privileging. Demonstrates proficiency in follow-up policies, procedures and practices. Signs off on lab/imaging reports and creates follow-up plans for abnormal lab or imaging results, in adherence with affiliate protocols. Documents clearly and comprehensively so that the CCT and health center staff can easily carry out notifications and reminders for each follow-up plan. Supports the agency’s mission and all the sexual and reproductive health services offered including, but not limited to, family planning services, preventative health screenings, testing and treatment of STI’s, birth control methods, gender affirming care and abortion services. Completes all responsibilities according to established protocols, policies, and standard practices in the areas of customer service, quality assurance and regulatory compliance programs such as HIPAA (Health Information Portability & Accountability Act), OSHA (Occupational Safety & Health Administration) and CLIA (Clinical Laboratory Improvement Act). Maintains active licenses and certifications as required to fulfill the role, including but not limited to state medical license, DEA license, and BLS Certification. Complete reappointment process with Credentialing every three years. Supports quality improvement activities such as performing chart audits. Assist the Practice Manager with implementing new policies and initiatives, such as Press Ganey and template updates, while maintaining a positive attitude. Monitor overall workflow and assess the need for overtime based on staff requirements, communicating adjustments to the Practice Manager. Professional Development and Quality Improvement Stay current on electronic health record policies and procedures. Support and participate in quality improvement initiatives. Attend required meetings and actively participate in committees and professional development activities. Ensure accountability for professional competency, demonstrating critical thinking and knowledge to deliver optimal patient care. Provider and Staff Support Act as the primary contact for providers on-site, addressing concerns and communicating requests to the Practice Manager when necessary. Participate in hiring and onboarding new staff, ensuring they are adequately prepared for their roles. Aid in developing and revising patient care plans under the direction of an MD or APRN. Perform patient-related activities such as medication administration, diagnostic testing, therapeutic treatments, medication renewals, and prior authorizations. Other Duties Document nursing interventions and patient responses to care accurately and communicate this information to the appropriate healthcare team members. Conduct follow-up tasks, including post-operative calls, and handle triage phone calls efficiently. Perform additional automated functions and other duties as assigned to support the delivery of safe, efficient, and effective patient care. Knowledge, Abilities Knowledge of the principles and skills needed for practical nursing to provide patient care and treatment. Ability to communicate clearly with individuals of all levels and establish and maintain effective working relationships with providers, staff, patients and the public. Knowledge of examination, diagnostic and treatment room procedures. Knowledge of common safety hazards and precautions to establish a safe work environment. Minimum Requirements Graduation from an accredited School of Nursing. Valid and unrestricted Connecticut State Registered Nurse License One year medical office experience, including phlebotomy and EKG. Environmental Working Conditions Exposure to bodily fluids, illness and disease Physical And Mental Demands Requires full range of body motion, including handling and lifting patients; manual dexterity; eye-hand coordination. Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed; participates in on-call rotation for clinical emergencies and other clinical/facility issues. Assisting the DNS in on-going training of nursing staff to support clinical services and to meet and exceed established clinical competency levels; monitors and assists in the training of specialty unit personnel to maintain active competencies and/or certifications required for the specific unit. They must also have: Must have at least 2 years of increased responsibility in a supervisory position in long term care and/or rehabilitation facility or hospital setting or equivalent nursing education degree program. Six (6) months of training experience in rehabilitative and restorative nursing practices preferred. IV Certification required. Experience with state survey process, survey tag numbers, and quality measures preferred.
