Description RN/Case Manager MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting. Licensing and Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected. Description The Nurse Manager (NM) is responsible for the management of nursing practice and operations on a designated unit/service setting such as inpatient, procedural, emergency department or ambulatory care. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. This individual works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit setting maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Responsibilities Patient Care Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. Demonstrates proficiency in all aspects of Care Checks and role models expected behaviors. Ensures the integration of Care Checks into clinical activities to enhance Relationship Centered Care validates staff members Care Checks on all patients by direct observation and patient feedback. Assesses staff competency in Care Checks/PHR and provides consistent meaningful feedback with more focused feedback as needed. Conducts rounding of patients on a daily basis (Monday-Friday.) Conducts rounding on staff on a weekly basis. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. Monitors institutional information technology infrastructure and corrects gaps in clinical nursing documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. Prioritizes recruitment and retention strategies in accordance with system and local hospital retention goals. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations in all clinical specialties. Implements and monitors unit salary and supply budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Monitors availability and maintenance of nursing equipment and supplies Maintains position control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Evaluates all newly hired/transferred staff and participates in the decision about passing the probationary period. Participates in the ongoing and completes the annual performance appraisals for all staff. Completes performance improvement plans of employees as needed and shares feedback with the Associate Directors/Directors and Senior Directors accordingly. Interprets and implements the associated collective bargaining agreements and System and Hospital policies and procedures. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. Acts as a management witness in disciplinary activities. Insures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. . Description Responsible for the care activities and goal achievements of assigned patients by assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient's response to care. Discharge planning consistent with the patient’s status and reflects inter-departmental and inter-agency communication. Communicates information to the patient based on the patient’s level of awareness and readiness to learn. Supports orientation of new staff members by observing professional behaviors, providing feedback to the staff member and by acting as a preceptor, role model and a resource person. Functions as a professional role model and mentor for students and volunteers. Patient care assignments reflecting an awareness of patient needs and skill level of personnel. Organization of provision of care to meets priority needs of patients and unit. Communicates condition of patients and unit requiring additional intervention to the leadership person in the relevant discipline. Participates in the development of other staff members. Communicates with peers in an appraisal of practice as it relates to self and other nursing personnel. Assumes accountability and responsibility for completion and quality of assigned tasks. Identifies and communicates to immediate supervisor the need for further staff education. Qualifications Graduation from an accredited Nursing program. Bachelor’s Degree in Nursing. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Documented previous experience or educational preparation to support credentials.
