This role provides professional nursing care for patients in an inpatient hospital setting. MINIMUM QUALIFICATIONS : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Overview Meritus Health is building a high-autonomy house-calls program for patients who need wound care and longitudinal primary care in their homes and in our Home Health clinic. We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model. Role Summary In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans. Key Responsibilities Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions. Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies. Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations. Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting. Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans.
