Essential Functions & Responsibilities Clinical Care & Care Coordination Perform comprehensive assessments, reassessments, and ongoing monitoring to identify needs, changes in condition, and appropriate level of care; update Plans of Care accordingly. Triage participant and staff calls to determine priority and route/escalate appropriately.
