Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. Responsibilities The Patient Throughput Coordinator will work with multiple entities to find and surmount the barriers to discharge of patient. This position acts as the designated liaison who facilitates and coordinates the transfer of patients into and within University Hospital. ESC is pioneering the first ever facility designed to care for young adults with severe and chronic medical needs who have aged out of pediatric services. Our future young adult center will be located on our White Plains campus and is slated to open in 2028. Reporting to the Administrator of the young adult center, the Chief Nursing Officer provides strategic and operational leadership to plan, organize and direct the daily operations in accordance with regulatory guidelines and standards. RESPONSIBILITIES Strategic Planning: Participate in ESC Strategic Planning Committee in partnership with the Board of Directors and be accountable to meeting the agreed upon metrics for monitoring success. Establish and implement an interdisciplinary vision that provides inventive, holistic care and education of unparalleled quality to transform the lives of young adults with medical complexities. Regulatory Compliance: Drive quality care by setting and upholding high standards of nursing practice, ensuring every young adult receives personalized, top-tier care in alignment with regulatory guidelines. Operational Leadership: In collaboration with Administrator and Chief Medical Officer, lead an interdisciplinary team in the development of an innovative care model that will include enhanced programming, clinical pathways and new staffing patterns for our young adult center. Be a voice for nursing by representing ESC in professional nursing organizations and committees advocating for excellence for young adults with medically complex conditions, presenting and publishing best practices at state and national conferences. Solve problems with compassion by addressing and resolving concerns from residents, families, and staff with empathy and professionalism. Focus on Quality and Safety: Lead a rigorous Just Safety Culture with accountability, empathy, and transparency. Review quality data to identify safety, infection control and regulatory trends to improve nursing practices. Collaborate across teams by working closely with other departments and community partners to enhance resident care and create opportunities for staff development. Promote employee engagement by active listening and responding to concerns, and involving staff in decision making. Qualifications Education New York State RN License and Registration Certificate BCLS certification Master's Degree or Doctorate in Nursing Practice Experience Minimum of ten years of nursing experience with 5-7 progressive leadership experience in Nursing Administration with demonstrated effective leadership skills. Excellent interpersonal and communication skills (written and oral). Effective management skills necessary to organize direct and evaluate the activities of patient care off shift operations. Ability to apply and utilize the balanced scorecard measures (customer service, financial, innovation and growth, and performance improvement). Critical thinking and analytic skills necessary for problem solving and clinical decision-making. Public speaking experience and the ability to deliver impactful presentations. Responsibilities The primary purpose of the Staff Nurse position is to be accountable for the care activities, safety and goal achievements of assigned patients through assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient’s response to the care. These responsibilities also include patient education and discharge planning. The Registered Professional Nurse places emphasis on mobilizing patients and preventing skin break down, falls and implements theappropriate measures to achieve this. The Registered Professional Nurse is a member of a designated care team and collaborates,coordinates and guides team members in planning patient care in a safe environment and achieving patient care goals according to thepatient/family developmental stage and cultural expectations. Qualifications Licensed as a Registered Nurse in the State of New Jersey required. BSN preferred. Adult Medical/Surgical experience preferred. ACLS and BLS required. Must maintain valid certification by the American Heart Association in basic life support (BLS). Ability to employ problem solving techniques and critical thinking. Effective interpersonal skills. Ability to lift and move patients with/without reasonable accommodations. Ability to organize care/activities of multiple patients. Successful completion of a Progressive Care Course or Progressive Care Certification, preferred but must be achieved within first six (6) months of hire. Phlebotomy Certification or IV Certification is preferred but must be achieved within first six (6) months of hire. Proficiency in the English language is required. FIT Testing required. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Registered Nurse within the state of employment. Responsibilities The primary purpose of the Advanced Practice Nurse (APN) position is to provide high quality patient care. The APN utilizes advanced practice nursing expertise to provide care to patients in the Emergency Department. Other responsibilities for the APN include, but are not limited to, participation in staff orientation, consultation and evaluation for patients, families and health care team members. Works collaboratively with Physicians, Nursing Director and Managers to identify patient care and staff development needs and quality improvement opportunities. Qualifications Certified as an Advanced Practice Nurse in the State of New Jersey. Three (3) years of nursing experience in Emergency Department, Critical Care or Medical/Surgical required. One (1) year of experience as an Advanced Practice Nurse in Emergency, Critical Care, Adult or Family preferred. Demonstrates effective leadership competencies to be carried out in the day-to-day activities of the department. BLS (CPR) and ACLS required. Computer literacy required.
