Performs Central Venous Line (CVL) site observation/inspection; venous access device dressing, cap change and flushing venous access device; IV medication and fluid administration, IV pushes (per procedure) and venipuncture. May not make any changes to infusion pump including, but not limited to, rate changes and IV bag changes. Documents per policy and procedure in medical record. Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. This unit also specializes in peritoneal dialysis while also providing treatments such as cardiac monitoring, IV therapy, nutritional support, diagnostic testing and spiritual support. How You’ll Benefit Our Center for Workplace Violence Prevention and Caregiver Well-Being sets caregiver safety as a priority, identifying, reporting and responding to incidents and providing safety resources. A caregiver in this position works PRN (as needed) variable shifts. A caregiver who excels in this role will: Provide direct nursing care. Implement and monitor patient care plans. Monitor, record and communicate patients’ conditions. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Note and carry out physician and nursing orders. Assess and coordinate patients’ discharge needs. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited School of Professional Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross External candidates: two years of recent related nursing experience Internal candidates with less than two years of experience, but are post-orientation/90-day probationary period, may be considered with CNO level review and approval Licensed military RN experience will be considered Successfully pass all prescreen competency assessments Preferred qualifications for the ideal future caregiver include: Bachelor of science in nursing (BSN) Physical Requirements: Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extended periods of time. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and irregular hours. Exposure to communicable diseases and/or body fluids. Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
