POSITION OVERVIEW The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement. Key Responsibilities Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents’ physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers. Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases. Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents. Ensure that MDS notes are informative and descriptive of the services provided and of the residents’ response to the service. Evaluate and implement recommendations from established committees as they pertain to the assessment and/or care plan functions of the health campus.
