Caregivers and patients work together to achieve the best possible health outcomes. A caregiver in this position works Monday through Friday from 8:00am to 5:30pm. Once a week there will be coverage needed until 7:30pm. Rotations include 1 weekend a month from 8:00am to 12:00pm. A caregiver who excels in this role will: Assist with performing screenings and treatments by gathering and updating information related to health maintenance, identifying medication changes, updating allergy information, collecting specimens, performing tests and documenting patient care. Monitor and communicate changes in patient condition. Maintain equipment, medical supplies and examination areas. Perform administrative/clerical duties, such as filing, reception, scheduling, data entry and patient registration. Minimum qualifications for the ideal future caregiver include: Graduate from an approved medical assisting program OR military training as a hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) Completion of a clinical externship OR extensive clinical experience Basic Life Support (BLS) certification through the American Heart Association (AHA). Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner. This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Urgent Care RN staff in these roles will help foster positive and effective working relations with senior leadership and management, local market management and business ancillary departments for the purpose of achieving the company's vision, mission and objectives.
