Home Therapy position Must be willing to assist at other clinics as needed PURPOSE AND SCOPE: The professional registered nurse Home Therapies RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. Performs assessment and identifies barriers of the Home Dialysis candidate’s home environment and partner/ family readiness and ability to perform dialysis treatments in the home. Trains home dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a formal standardized Home Dialysis Training Program. Participates in education and quality improvement projects at the facility and area level as directed by Supervisor. May serve as a Preceptor to new employees. Required to complete CAP requirements to either maintain or advance. Performs all other duties as assigned by Supervisor. Position Highlights: Status: FULL TIME Day Shift 7am-3:30pm What You’ll Do: Provide nursing care to residents in accordance with established care plans, policies, and procedures. Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition. Administer medications and treatments as prescribed and document appropriately. Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers. Support health promotion, disease prevention, and wellness initiatives within the community. Ensure accurate, timely, and compliant clinical documentation. Assist with care planning, service plan updates, and resident evaluations as required. Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery. Respond to resident concerns, incidents, and emergencies using sound clinical judgment. Support training and guidance for care staff as appropriate. Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services. Participate in quality assurance initiatives, audits, and compliance activities as needed. Perform other duties as assigned. Qualifications: Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations Minimum of two (2) years of Licensed Nurse experience required. Experience in assisted living, long-term care, or a skilled nursing environment preferred. Strong clinical judgment, problem-solving, and decision-making skills. Effective organizational, time management, and communication skills. Ability to manage multiple priorities and delegate appropriately. Demonstrated commitment to seniors and person-centered care. Ability to work nights, weekends, and flexible schedules as required by the community. Proficiency with electronic documentation systems and basic computer applications. Ability to respond to medical emergencies. Exposure to bloodborne pathogens and infectious disease. Ability to lift or assist residents as required by care plans.
