Shifts Available Day Shift M-F 6am - 6pm Night shift 6pm - 6am Every other weekend and on weeknights every other week Same day pay available. Must be on the Nurse Aide Registry in Kentucky POSITION PURPOSE: . Ensures that resident's personal care needs are being met in accordance with Residents' wishes and Care Plan Objectives. ESSENTIAL FUNCTIONS OF POSITION: Answers call lights, to determine residents' need and ensures call lights are within reach of resident Gives direct resident care such as bathing, dressing, and grooming of residents. Assists residents as necessary in their activities of daily living Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs. Follows infection control guidelines. Reviews and implements nursing needs as outlined by Resident Care Plan or nurse managers. Transports residents or assist residents with mobility needs. Turns and repositions bedfast residents, alone or with assistance. Takes and records temp, B/P, Pulse, resp rate., Ht., Wt., and food fluid intake and output, as directed. Participates in and receives nursing report upon reporting and departing to/from work. Reports all changes in residents' conditions to supervisor. Maintains cleanliness of residents' units, central bathing area, treatment and supply rooms, workrooms, and linen closet. Accurately records all care as directed. Participates and attends periodic in-services and staff meetings. Understands and adheres to the guidelines of "Resident Rights". Performs additional duties as assigned. EDUCATION / REQUIREMENTS: . Prior experience in LTC preferred but not required. . Able to effectively communicate with residents and nursing staff. . Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program. . Good standing in the KY registry is required.

Position Purpose: Be in charge of a CNA ensuring they are able to provide the highest quality of care for residents. Under the supervision of nursing department, the medical support staff CMT will have the following requirements, duties, and responsibilities: Accurately safely prepare, administer, and document the oral and/or topical medications that are commonly used in the facility that are ordered for participant use. Ensure that no medication is administered without a written, signed and dated order from an attending licensed physician. Verify that adequate supply of stock medications, supplies, and equipment is on hand to meet the nursing needs of the participants. Report needs to supervisor. Verify that narcotic records are accurate for your shift. Follow facility policies and procedures regarding the destruction of medications. Provide medical transportation of residents to appointments. Obtain the necessary paperwork from the Physician/clinician and provide liaison work back to manager and facility. Schedule medical appointments, as requested. Obtain medication from the pharmacy and distribute to residences. Review medication inventory, documentation and expirations. Performs other related duties and activities as required, including providing DSP duties SUPERVISORY RESPONSIBILITIES None Minimum Knowledge and Skills required for the Job The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job. Education and Experience: High School Diploma or GED Equivalent Experienced direct care provider and CMT with a minimum of one year of positive performance in healthcare. Meets all requirements of direct care position. Certificates, Licenses, and Registrations: Active Kentucky Certified Medication Technician Other Skills and Abilities: Must be 21 years of age to be considered for role Other Requirements: Travel as part of role with driving record that meets Sevita standards Physical Requirements: Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
