JOB SUMMARY Assists in examination and treatment of patients under the direction of a physician and/or advanced practitioner, including preparing patients to be seen by the physician and/or advanced practitioner, and perform specific procedures with appropriate orientation and supervision. Assists with patient phone calls, prescription refills, referrals, and prior authorizations. Interviews patients, measures vital signs (i.e. pulse rate, temperature, blood pressure, weight and height) and records information in patients’ charts. May be required to collect blood samples, tissue or other laboratory specimens and prepare them for testing. Explains treatment procedures, medications, diets and physician’s instructions to patients. Prepares treatment rooms for patient examinations, keeping the rooms neat and clean. Familiar with standard concepts, practices and procedures related to the practice. Follows standard procedures and/or written instructions to perform job duties. Assists with business operations, clerical, medical records management, reception, and patient scheduling as needed. Demonstrates teamwork to ensure achievement of team, individual, and corporate goals. Provides support to team members and performs other duties as required. JOB QUALIFICATIONS Education: High school graduate or equivalent. Licensure: Current BLS (Basic Life Support) certification required from the American Heart Association or the American Red Cross. Graduated from a Medical Assistant program, accredited by the Commission on accreditation of Allied Health Education Program preferred. Test and pass the National Certification exam (AAMA, NHA, NCCT, AMT, NAHP) to be certified as Medical Assistant required. Experience: 1-2 years’ experience in a clinical setting is preferred. Position: Advanced Practice Provider (NP/PA) Monogram Health is seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high‑quality in‑home care to patients living with multiple chronic conditions, including diabetes, hypertension, chronic kidney disease, heart failure, COPD, depression, and other complex health needs. This role works within an integrated multidisciplinary care team (Clinical, Behavioral Health, Pharmacy, Administrative), establishing long-term patient relationships and providing continuity of care to complex patients in their homes and via telehealth.
