Obtains specimens as assigned (e.g., urine, stool, sputum). Weighs patient and records weight. Assists patient with activities of daily living including skin care (e.g., bathes, dresses, helps patients ambulate). Records oral intake and urinary output. Performs assigned treatments (e.g., R.O.M., Dayroom Monitoring, Courtesy Cart). Communicates effectively with Charge Nurse/Primary Nurse regarding patients’ needs and requests. Orients patient to environment and develops rapport with patients, family and significant others. B. PATIENT CARE RELATED DUTIES Keeps patient unit and non-patient unit clean (e.g., wipes tables, neatens storage rooms, pantries and other areas). Prepares bedside units for new admissions (e.g., stocks with bedpan, tissues, water, cups). Apply principles of infection control and safety in the work environment. Reports significant unit problems to charge nurse. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Promotes adherence to the Patient's Bill of Rights. Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations. Understands and adheres to the Medical Center’s Code of Conduct. Other Duties : Attends the required 12 hours of in-service education class per year including resident rights, advance directives, and abuse & prevention. Effectively utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy. Participates in orientation of nursing assistants. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. PURPOSE AND SCOPE: Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provides day to day direction and supervision to assigned direct patient care staff. Communicates with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans. Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. Supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. PRINCIPAL DUTIES AND RESPONSIBILITIES: General and Staff Related: Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Clinical Manager and initiate as appropriate. Responsible for accurately documenting in an EMR system in accordance with licensure and VMG policy and procedures. Maintains close working relationships with all practice staff to ensure proper workflow, care and treatment. o (BA) Performs a variety of essential front desk activities that include greeting, registering, ensuring insurance compliance and collecting appropriate payment at time of service. Responsible for maintaining the daily patient schedule and answers all incoming calls that results in scheduling an appointment, medication refills or pass along inquiries from a clinical or billing nature, in a courteous, caring and respective manner. Coordinates with the clinical staff on any special needs or requirements and ensuring proper patient flow. Supports the Practice and professional staff as directed or requested by Practice Manager. Education High school diploma or equivalent and completion of a formal Medical Assistant program that is recognized by the State of New Jersey. Certification required. Completion of formal coursework in office management, database management or business. Computer l Experience Previous MA experience in a clinic, physician's office or outpatient setting preferred. Six (6) months of previous customer service/data entry, administrative/office management/clerical experience required. Skills Current and valid certification as a certified medical assistant or registered medical assistant recognized by the Board of Examiners such as American Medical Technology (AMT), American Association of Medical Assistants (AAMA), National Center for Competency Testing (NCCT) required. BCLS certification preferred upon hire, will be required. Understanding of the operations of a medical practice preferred. Strong orientation towards the provisions of high quality, cost effective and comprehensive medical care. Ability to use effective organizational and time management skills to set priorities and respond quickly to urgent/emergent situations.
