The Pediatric Hematology / Oncology Department at the Duke Children's Health Center is active in delivering exemplary care for each patient to ensure an excellent patient experience. The Pediatric Hematology / Oncology RN Care Coordinator will provide triage support and care coordination for this high-acuity patient population in collaboration with dynamic multidisciplinary teams. The role establishes rapport with patients and families by phone, coordinates services, and provides education and nursing support to patients and families in and out of the clinic setting. Coordinate internal and external referrals, future appointments, post-hospitalization care, home discharge planning, and transitions with inpatient and hospital care coordination teams. Support patients and caregivers with information about service costs, insurance coverage, payment options, and access to continuing care resources. What you will need BSN required. 3 years of nursing experience required. Current or compact RN licensure in the state of North Carolina required. Solid background in pediatric nursing; hematology/oncology experience preferred. Experience with telephone triage or managed care coordination. Strong oral and written communication skills and software navigation abilities; EPIC experience preferred. Good organizational skills. What you can expect Location: 2301 Erwin Rd, Durham, NC 27710 The Peds Transplant & Cellular Therapy unit, DCT 4A, is comprised of 17 beds and serves a blended patient population of pediatric transplant, cell therapy and general pediatric oncology, with an emphasis on high-risk oncology. Our team is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time. The DCT 4A team cares for oncology and sickle cell patients, as well as patients with metabolic and genetic disorders. Nurses in this unit work as part of a multidisciplinary team and have the opportunity to participate in rounds on their patients. Nursing care is patient- and family-centered, with primary nursing teams delivering care alongside members of various other disciplines. The nurse-to-patient ratio is 1:1 or 1:3, depending on acuity, and the unit is staffed with a free-standing charge nurse and Health Unit Coordinator 24/7. Description Nursing Assistant needed for Cardiac Intensive Care Unit (CICU). This unit provides patient and family centered care with the interdisciplinary healthcare team to medical cardiology patients. The CICU specializes in treatment of acute and chronic heart failure, cardiogenic shock, post cardiac arrest, STEMI and NSTEMI, noninvasive valve repair (TAVR), and vascular patients.
