Patient care needs are identified through interdisciplinary assessments, team care planning, and a patient classification system. A multi-disciplinary approach is used throughout the continuum of service with a special focus on pain management, medications, self-care, nutrition, and education. The team also provides special procedures including chemotherapy administration and monitoring, pain management, metabolic support, patient and caregiver education and support and, palliative care. We also provide quality care to patients undergoing chemotherapy, surgery, and radiation and for patients with leukemia, lymphoma, and multiple myeloma stem cell/bone marrow transplantation (BMT). We also have medical and telemetry patients and nurses are trained for various research protocols as they administer the medication for the research study. As a Registered Nurse IV on 4South, you will assume the responsibility and accountability for the supervision and application of the nursing process throughout the delivery of patient care. Right at Home Long Beach is looking for an experienced CNA or LVN to provide hands-on, in-home training to our caregivers. This is a per-visit, hourly position — you'll be scheduled as needed to go into clients' homes and coach caregivers through the hands-on skills that are hardest to learn from a manual, with a particular focus on safe transfers and care for clients with limited mobility. The Caregiver Skills Trainer is responsible for creating extraordinary client and family experiences and ultimately trains our caregivers in the home. Provide one-on-one, in-home training to caregivers on safe transfer techniques, including Hoyer lift (mechanical/full body lift) transfers and gait belt transfers. Train caregivers on the care of bed-bound clients: safe repositioning and turning, use of draw sheets, skin and pressure injury (bedsore) prevention, and proper body alignment. Demonstrate and correct proper body mechanics to protect both caregiver and client during transfers, ambulation, and repositioning. • Coach caregivers on two-person transfers, sit-to-stand and pivot transfers, and wheelchair/bed-to-chair transfers. Train on personal care for clients with limited mobility: bathing, toileting/incontinence care, dressing, and oral hygiene in bed. • Review infection control and standard precautions (hand hygiene, PPE use, catheter and wound-adjacent care). Train on recognizing and reporting changes in condition — skin breakdown, signs of infection, falls risk, and changes in mental status. Observe caregivers performing skills in real time, provide constructive feedback, and document competency/training completion for the client's file. Communicate observations and concerns to the office and clinical team promptly. Model RAH's caregiving standards and represent the agency professionally in clients' homes. The desired candidate will possess the following: Active California CNA or LVN license/certification in good standing AND HCA required. Minimum 3 years of hands-on clinical or home care experience, including transfer techniques and mobility assistance. Demonstrated proficiency with Hoyer lift and gait belt transfers, and care of bed-bound or non-ambulatory clients. Current CPR/BLS certification. Comfortable teaching and coaching others — patient, clear communicator who can break skills down step by step. Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
